Do each of my QuietGrowth portfolios constitute separate instances of Managed Discretionary Accounts?

No. You can invest in more than one QuietGrowth portfolio. All the portfolios in which you opt to invest will become a part of your Investment Program set out by the Managed Discretionary Account (MDA) created for you. So, all your QuietGrowth portfolios in your QuietGrowth account are managed within one single MDA.
